This is due NOW. The Sponsor Verification Form is for non-St. Ursula parishioners and is due NOW. Your Confirmation Candidate has the form. In accordance with Canon Law, sponsors must be at least sixteen (16) years of age and practicing Catholics who have received the Sacraments of Baptism, Eucharist, and Confirmation. Parents cannot sponsor their own children.

I recognize that as a sponsor for this Sacrament, I am committed to keep God’s commandments as Jesus has taught us: to worship God through prayer and faithfully attend Sunday Mass and to also strive to translate my faith into good examples and works.
