Please fill up this form per child. Cancellation of order on the same day will be charged to the student. Payments (cash, check under CID Food Services may be dropped at the back lot's drop box). Credit card payment is acceptable, please call Sheren at (671) 489-1511 at 2pm to process via telephone transaction. Please understand if she is not able to pick up calls during busy school hours.

We would like to inform you that we will be implementing Cash basis only (except for those with remaining funds). If account is depleted, we will no longer be able to charge anymore.
