COVID reporting form for in-person attendees to AABA 2023
We are only using the information submitted to provide support and to be transparent about the prevalence of COVID at the 2023 conference. We recognize that health information, like test status, is private and sensitive. Only a small number of BAI staff will have access to your personal information.   The intent of this form is to gather information from those who believe that they contracted COVID as a result of the AABA 2023 meeting in Reno.

AABA is not providing contact tracing.  We encourage you to reach out to known close contacts or to reach out to the AABA COVID response line if you have questions or would like help.
