Please complete the following survey for your Grade 4 child only. If there are siblings in your household, you will need to complete each child's grade-specific survey posted on their Google classrooms. Please submit your survey by Tuesday June 16th.

Does your Grade 4 child have access to a technology device solely for his/her use at home (not shared with other members of the household)? Please check the following box that applies to your current at home remote learning environment. A device is defined as a computing device, such as a laptop, desktop, Chromebook, iPad, or full-size tablet. “Device,” for the purposes of this survey, is NOT a phone or mini-tablet. *

My Grade 4 child DOES have access to a device solely for his/her use for remote learning purposes(not shared with others).

My Grade 4 child shares a device with other children or adults in the household during hours the student would need the device for learning purposes.

My Grade 4 child does not have any access to a technology device, as defined above, to aid in his/her participation in remote learning.
