CISD Prekindergarten Withdrawal Form 
Parents/Guardians will fill out this form when informing CISD of student(s) withdrawal from the CISD High-Quality Prekindergarten Program. 

Once your student is withdrawn from the prekindergarten program, there is no guarantee a spot will be available to reenroll.

It the student has a Tuition-Based spot with auto-payment from banking account, this form must be filled out 30 days prior to the next scheduled tuition payment. This is stated on the CISD prekindergarten website/tuition, and in the Tuition-Based Prekindergarten Parent Agreement filled out during enrollment.
