If your child(ren) will not participate in the onsite testing coordinated by AFA, complete the form below no later than 48 hours prior to the testing event to communicate your alternative testing plan.
Reasons for alternative testing are: your family's health insurance does not accept testing from third-party providers, the child is in half-half day or 2-day PreKindergarten for (in-school testing dates), your child needs additional parental support to complete testing, your child tested positive for COVID after December 17th.
Alternate Testing must be conducted locally (DMV area) and occur 24-72 hours prior to return to school (between Thursday, January 20th - Sunday, January 23rd). PCR testing must be conducted by a medical professional at a testing site. Home tests will not be accepted.
