This form will be sent to WisER Driving School (formerly B & L Driving School) to register your child and provide the school with contact information.  The $400 cost for the class is required to reserve a seat in the desired class. Payment information is available on our website, bldrivingschool.weebly.com. DO NOT COMPLETE MULTIPLE FORMS.  THAT WILL ONLY DELAY PROCESSING! You may edit your responses.

Refund Policy:  If a cancellation is received 10 days before the class start date you will receive a refund of $280; a cancellation received 5 - 9 days before the class start date you will receive a refund of $200; a cancellation received less than 5 days before the class start date will NOT receive a refund of any amount. *
