I acknowledge that working with dogs, including training, socializing, grooming, walking, and other activities, involves inherent risks. I accept full responsibility for myself and my dog(s) during and after these activities with Macatawa Dog Training LLC, provided that reasonable care and diligence are exercised. I will not hold Macatawa Dog Training LLC, its affiliates, or property owners liable for any injuries or misfortunes that may occur to me or my dog(s) during these activities. I understand that risks such as tripping, falling, being knocked down or jumped on, and being bitten or scratched are part of these activities.

I also acknowledge that by enrolling my dog(s) in Macatawa Dog Training LLC’s programs, I am confirming that my dog(s) are in good health and have received the required vaccinations (or their equivalent as approved by my veterinarian). I grant Macatawa Dog Training LLC permission to request and obtain my dog(s)’ veterinary records from my veterinarian for any reason related to my dog(s)’ participation in their programs. Should my dog become injured or ill, Macatawa Dog Training LLC is authorized to contact my designated veterinarian as indicated on the training information form. In the event of an emergency where my veterinarian is unavailable, Macatawa Dog Training LLC may take my dog(s) to the nearest animal hospital for immediate care. I agree to fully reimburse Macatawa Dog Training LLC for any medical expenses incurred while my dog(s) are under their supervision. Macatawa Dog Training LLC will not be responsible for any decisions made on my dog’s behalf.

I understand and agree that all services provided by Macatawa Dog Training LLC are non-refundable. I acknowledge that this policy applies to all payments made for training, socializing, grooming, walking, and other services.

I understand that Macatawa Dog Training LLC does not guarantee specific results from the training programs. Dog behavior can be unpredictable and influenced by factors beyond the trainer’s control. I acknowledge that achieving desired outcomes depends on various factors, including the consistent application of training techniques.

I also acknowledge that I am responsible for implementing and maintaining the training techniques at home as advised by Macatawa Dog Training LLC. My active participation and follow-through with these techniques are essential to the success of the training.

I hereby release and agree to hold harmless Macatawa Dog Training LLC, Erin Smith, its affiliates, and property owners from any and all liabilities, claims, suits, actions, losses, injuries, or damages of any kind that may arise from my dog(s) or their activities. I agree to fully indemnify these parties for any such liabilities or damages. I understand that I am financially responsible for any damage or harm caused by my dog(s) during training, socializing, grooming, or walking.

I commit to asking questions and using my judgment to ensure I am comfortable with the activities and procedures. I understand that I am not required to participate in any activity or allow any procedure if I am not comfortable with it.

I confirm that I have read and understand the rules, regulations, and terms of this agreement. I agree to abide by these terms and conditions and verify the accuracy of the information provided in the release form completed by me. I acknowledge that some programs may not be suitable for all dogs and that Macatawa Dog Training LLC reserves the right to remove or refuse any dog from its programs. I also understand that photos of my pet may be used for social media and marketing purposes.

This agreement shall be governed by and construed in accordance with the laws of the State of Michigan. Any disputes arising under or related to this agreement shall be resolved in the courts of Michigan. If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall continue to be in full force and effect.

I agree not to disclose, copy, or use any proprietary training methods, techniques, or business practices of Macatawa Dog Training LLC for any purpose other than as expressly authorized by Macatawa Dog Training LLC. This obligation of confidentiality shall remain in effect indefinitely, even after the termination of this agreement.
