Registrants for all Villarreal Soccer Academy programs must accept the following Participation Sportsmanship Commitment, Waiver of Liability, and Privacy Policy in order to submit their registration form. In consideration for my voluntary participation in organized soccer activities through Villarreal Alberta Academy, I hereby willfully acknowledge to my understanding and agreement that: COVID 19 RULES/REGULATIONS Players are to stay home if they are experiencing symptoms including cough, fever, shortness of breath, runny nose or sore throat and/or are required to isolate or quarantine. Please do not attend your technical training session if you are experiencing these symptoms or are sick. An inherent risk of exposure to COVID-19 exist in any public place where people are present. COVID-19 is extremely contagious and can lead to severe illness or death. Senior citizens and guests with underlying medical conditions are especially vulnerable. By attending an indoor facility with other individuals you are voluntarily assuming all risks related to exposure to COVID-19. COVID-19 is an evolving situation and Villarreal Soccer Academy's guidelines may change without notice, all while in accordance
with AHS and ASA rules and regulations.

UNIFORM/SOCCER SETS
All players are expected to be dressed in Villarreal Alberta Academy soccer sets, which include: Villarreal Joma jersey, Villarreal Joma shorts and Joma socks. Cost is $150 (gst included) for each soccer set.
Because of the transition in soccer sets after signing the partnership with Villarreal CF, we are only selling one set per player at this time. Players will be able to purchase more than one set starting in the fall of 2022.

PAYMENT
Space is limited per camp. Player will be added to the roster once FULL PAYMENT is made BEFORE the first day of camp. Payment will guarantee your spot in the academy.
Once payment for camp is made, Villarreal Alberta Academy will honour any credits if we cancel practice on our end. If you decide not to attend or can not attend practice for reasons of your own, Villarreal Alberta Academy will not credit back that day missed.

Villarreal Alberta Academy is responsible to abide by all the ASA and AHS rules and regulations. We will communicate any information the pertains to our players and camps.

For all participants: WE HEREBY AGREE THAT VILLARREAL ALBERTA ACADEMY MEMBERS, COACHES OR ANY OFFICERS SHALL NOT BE LIABLE FOR ANY INJURY OR LOSS WHICH MY CHILD MAY SUSTAIN WHILE PARTICIPATING IN ACTIVITIES OF ANY KIND WHETHER SPONSORED BY OR UNDER THE SUPERVISION OF THE PARTICIPANT AND WE AGREE TO INDEMNIFY AND TO HOLD HARMLESS ITS MEMBERS, COACHES, OFFICERS OR DESIGNATES OF ANY KIND FROM ANY CLAIM WHATSOEVER.

I will not compromise myself in such a way as to do harm to the academy or facilities, knowing that players may be dismissed from participation, with possible loss of payment or dues, for violent conduct or unsportsmanlike behavior on or off the field. I will exhibit good sportsmanship in all activities. I will conduct myself appropriately in interactions with youth, parents, coaches, and all Villarreal Alberta Academy administrators and officials. I will also report any observed violations of good sportsmanship that is witnessed to the league. I agree to pay for any and all damages to any property or indemnities caused by me willfully, negligently, or otherwise.

Soccer is a physical, contact sport that involves the risk of injury. I assume all risks and hazards associated with my participation in the sport. I am in proper physical condition to participate in soccer practices and games and have no illness, disease or existing injury or physical defect that would be aggravated by my participation. I will inform the league if this status changes. I further acknowledge that this risk may involve loss or damage to me or my property, including the risk of death, or other unforeseen consequences, including those which may be due to the unavailability of immediate emergency medical care. I will wear properly-fitted and appropriate shoes, and other protective equipment, as provided by soccer rules, and follow other safety guidelines issued by the league, to all events. I authorize my photograph, picture or likeness, and voice to appear in any documentary, promotion (including advertising), television, video or radio coverage of the Academy or tournaments, without compensation. Participants will never be tagged/identified on social media or the other above-mentioned sources without consent.

As a parent and natural guardian or legal guardian of the participant, I hereby agree to the foregoing Waiver of Liability and Release for, and on behalf of, the participant (player/minor) named above. I hereby bind myself, the minor, and all other assigns to the terms of the Waiver of Liability and Release. I represent and certify that I have the legal capacity and the authority to act for, and on behalf of, the minor in the execution of this Waiver of Liability and Release.
