The purpose of the survey is to capture participants’ attitudes and perceptions towards the COVID-19 vaccination. The questions asked in the survey can be answered by anyone who has received the vaccination, is yet to receive it or has chosen not to take it up. The aim is to understand the experiences and reasons for having/not having the vaccination and what could be done to increase uptake. This survey is being done in selected areas of Walsall.

As mentioned above, your participation is anonymous and voluntary. Information you give in this survey will remain confidential and responses will not identify you.

We will ask for data at the end of the survey like postcode, age, gender, sexual orientation, disability, ethnic background and nationality. These questions are optional and will not affect your participation. We ask for this data so we can understand more about who has participated in this survey.
