TPMS COVID Test Kit Opt Out Form

PUSD will be providing each student with a free take-home COVID test kit for the upcoming spring break. These test kits will be distributed on Thursday, April 6th in your students' Ram class. If you would prefer to NOT have your student receive a COVID take-home test kit, please complete the form below before 3pm on Wednesday, April 6th.
