F.O.O.D. Week 5 to Week 10 Registration Form
This fall 2021 we are lucky to have our new Community Chef Kay Miller provide us with fresh meal options for you and your family!

Registration for this program is going to be in batches. In this form you'll be registering for weeks 6 to 10 with pickup dates from Saturday November 20th to Saturday December 18th. Please use this form to confirm which weeks you want to register for meal pickup. Once the menu for the week has been set, you'll have the option to select the quantities of each dish you'd like to order for your household.

This is not the menu order form. Please complete this form and you will be contacted with the menu ordering form. You will be able to select 1 main dish, 1 side dish, and 1 dessert from that menu. Menus will usually include a meat or a vegetarian main dish, a healthy side dish, and a fun dessert. We'll be getting to taste a variety of dishes that reflect the cultural diversity of the West End neighbourhood.

This order form will remain open until November 17th. If you miss the registration for the first set of weekly meals, register for the wait list and you'll be informed of any vacant spaces in the program.

Order Periods are:

Week 6 - Pick up: November 20th
Week 7 - Pick up: November 27th
Week 8 - Pick up: December 4th
Week 9 - Pick up: December 11th
Week 10 - Pick up: December 18th


You'll be able to select your pick up location for your order on Saturdays from one of our two pick up locations:
- West End Community Centre - 21 imperial Road S
- ECOTT - 50 Fife Road (Lower Apartment Doors)
