The Hope Highway Adoption Application
                        Adoption Fees:         Puppies or Adult Dogs: $400*     
                                    Senior Dogs (8-11): $265.00    12 and up $160
                                    Dogs surrendered from breeders $600 (up to 7 yrs) 8 and over $425** 
                                    (Wisconsin state sales tax of 5.5% is included in prices)
                                

*Some treatments are not age appropriate, and puppies/dogs cannot have standard treatments and tests to be completely up to date on vaccinations (typically 8,12,16 weeks or about 4 weeks apart for first set of shots). Spay/neuter will need to be set up by the time the puppy is 6 months of age.

** There are additional medical costs associated to acquire these dogs. (Additional donations are appreciated to help cover the higher costs of these dogs)

Do you agree to have this dog up to date on vaccinations and treat any other health concerns in a timely manner after adopting? (Please note: This is a requirement and is non-negotiable, if you fail to do this, the dog will be forfeited back to The Hope Highway Dog Rescue.) *
We do require a 2 week foster to adopt program for adult dogs (4 months and older) before adoption will be finalized. Foster to adopt is a trial period to be sure the new dog is a good fit for the family and vice versa. If the dog is found to not be a good fit during this 2-week period, the adoption fee will be refunded upon returning the dog. ***Puppies (under 4 months) will not have a foster to adopt trial period. *** If the dog is surrendered or seized by the rescue after the 2-week trial or a puppy is returned any time after initial adoption, the adoption fee is forfeited to the rescue. The waiver and release form that was filled out to foster, remains in effect for the entire trial period. Do you understand and agree to the above statement? *
I understand that applying does not ensure approval and that failure to comply with the requirements of this application can result in forfeiture fostered through the Hope Highway Dog Rescue.  I certify the above information is correct and the information will be verified.  The rescue holds sole ownership until the application is approved, finalized, agrees the placement is the best fit for the dog and is paid in full by the adopter.  Do you understand and agree with the above statement?

Rescue animals are handled daily after they are taken out of situations they are rescued from, and as with most animals, they can react in an unpredictable way to sounds, sudden movements, certain people, or other animals that can result in injury.  By this waiver I/ my family agree to assume any risk and take full responsibility and waive claims of personal injury, death or personal property.  This waiver is for the duration of time working with the Hope Highway Dog Rescue.  Do you understand and agree to this statement?

Payment for adoption is due the day you pick up your new family member. The dogs will be up to dates on vaccines that are required for their age. Vaccines, spay/neuter (for puppies under 6 months), or any other health conditions and/or concerns that arise or are needed after you take your new dog home, is the responsibility of you, the adopter(s). If there is a health issue that The Hope Highway Dog Rescue deems was present before the adoption, we reserve the right to choose which veterinary clinic will be used to get the dog back to proper health. If you, the adopter(s), do not follow through with this contract, The Hope Highway Dog Rescue reserves the right to reclaim the dog(s) with the help of local authorities and the adoption fee will be forfeited to The Hope Highway Dog Rescue. If it is determined that you falsified your application to obtain the dog(s), The Hope Highway Dog Rescue reserves to right reclaim the dog(s) with the help of local authorities at any time and the adoption fee will be forfeited to The Hope Highway Dog Rescue.

If for any reason you must surrender or re-home the dog(s) that has been adopted from The Hope Highway Dog Rescue, you will contact the rescue to have the dog(s) re-homed or placed elsewhere with our help.
