We know it can be a difficult and traumatic time when you have to consider re-homing your cat or asking for assistance. To help our team at Felines of Chicago find the best possible forever home or foster (via Illinois licensed cat rescue), please answer the following questions as best as you can. All information will be used for rehoming purposes only and will not be sold. Information may be shared for rehoming purposes.

Is this a stray cat? *
If you answered yes to the cat being a stray or feral, have you had it scanned for a microchip? *
What is the maximum amount of time you are able to hold on to the cat(s)? *
Please explain, in detail, why you need to rehome your cat. *
What is the name of your companion animal? *
Please describe the breed, color of eyes and fur, and fur length (i.e, short, medium, long) of your cat. Please also give the approximate weight of your cat. *
How old are they? In addition, how long have you had them? *
is your cat originally from a cat rescue or shelter? If so, which one? If you are unsure, please ask talk to us about this. *
Is your cat microchipped? If so, whose information is on it? In addition, please provide microchip number if applicable. *
Has your cat lived in any other homes or places before living with you? For example, lived as a stray cat, neighbor’s cat you took in, another home (relative or stranger), municipality animal control, etc. *
Is your cat male or female? *
Are they spayed/neutered? *
Are they up to date on all vaccinations? *
Are they on monthly preventatives (i.e., flea, heartworm, tick prevention)? If so, which ones (e.g., Revolution brand)? *
Are they litterbox trained? If they have some issues with elimination or using the litter box, please elaborate using the Other option. *
Does your cat have any medical problems? If so, please describe in detail. *
Does your cat have any allergies to either food or medication? *
When was the last time your cat had a veterinary visit? *

Please write a biography of your cat. Things to include in detail are their personality (for example, are they independent or a lap cat? sassy or sweet? mischievous? etc), how vocal they are (are they chatty? a big purrer? quiet?) and their activity level (active? laid back?). Please include a funny/sweet anecdote as well as anything that you feel describes your cat and would help place them with their new family. *
Do they get along with other cats? *
Have they lived with any other animals of a different species such as birds or dogs? If so, did they get along? *
Please describe your home environment. For example, is it quiet or is there moderate noise? Are you usually home all the time or absent? Do you live with any other people? *
What are their favorite toys and activities? *
What type of food do they eat? Please include brand, wet or dry, frequency, and amount per day. *
What type of cat box and cat litter do they prefer? *
Do they have any dislikes? For example, do they hate being left alone, are they afraid of men or dislike children, do they have a certain spot where they do not like to be pet, stairs, etc *
What supplies and accessories will come with your cat? *
Are you able to transport your cat to their new home? Please indicate the distance you are willing to travel. *
How does your cat handle carriers, car rides? *
Please describe what you are looking for or would prefer in your cat's new home. *
Do you hope or intend to stay in touch with your pet in its new home? *
Some rescues may require you to pay a re-homing fee. They can range from $75-$125 depending on if the cat is up to date with vetting. Are you able to provide this? *
