This form should be completed by the person in the best position to evaluate the student's performance. After submitting this form, five copies of this evaluation form will be sent to: the academic course advisor, the student, the employer training coordinator, FoE Training Unit, and one to the submitter of this form (Evaluator).The Employer Supervisor (Evaluator) must 'reply to all' with confirmation of submission to  receipts to certify delivery of his/her submission.
This following information should be available to the evaluator of the COOP student before he/she starts filling out this form:
1- Academic Course Advisor Name & Email.
2- Student Name, KAU ID No., and Email.
You can ask either the student or his academic course Advisor to provide you with such information or you can contact us on: eng-training-team@kau.edu.sa 
According to this form, the student’s performance during the whole training period is evaluated with a maximum score of 25, noting that the student’s score less than 15 out of 25 is considered a failure in the training course regardless of the level of the final report submitted by the student.

نموذج التقييم النهائي للتدريب التعاوني هو نموذج يتم بموجبه تقييم مستوى الطالب أثناء فترة التدريب التعاوني بحد أعلى 25 درجة. مع العلم بأن حصول الطالب على درجة أقل من 15 من 25 يعتبر رسوب في مادة التدريب التعاوني بغض النظر عن مستوى التقرير النهائي الذي يقدمه الطالب. يعبئ هذا النموذج مباشرة من قبل المشرف على الطالب بجهة التدريب بناءا على اداء الطالب خلال كامل فترة التدريب. بمجرد ارسال هذا النموذج سوف ترسل نسخة  الى ايميل استاذ الماده ونسخة الى المشرف بالشركة. يجب على مشرف الشركة (المقيَم) "الرد على الكل" في الإيصالات المرسلة الى ايميله للتصديق على تسليم الطلب.
