Sage Summer Camp 2024 - Program Policies

Please check to agree to each statement and sign at the bottom to indicate you have read and agree to Sage Summer Camp's 2024 Policies.

I understand the Summer 2024 Hours of Operation will be 7:45am-5:45pm.

Middleton Sage Summer Camp 2024 is striving for maximum flexibility for families. I understand that enrollment will be week by week (can change week to week) and I will choose my child's schedule on this form.

I understand that since field trips, staffing, and snacks/materials are planning in advance, credits will only be issued when absences are submitted at least TWO WEEKS in advance, via the schedule request form on our website.

I understand that in order to withdraw my child from the Sage Summer Camp program, a Schedule Request Form must be submitted at least TWO WEEKS in advance of the intended final day of attendance. Final payment will be prorated for a partial billing cycle.
