This form should be submitted at least 2 WEEKS in advance of any pre-planned absence of any scheduled band function. These submissions will be kept on file throughout the season. Please note that missing rehearsal or an event for homework or studying is not an acceptable reason to miss. This form is the responsibility of THE STUDENT to submit.
