NIETOC SCHOLARSHIP CLAIM FORM
Congratulations! Your performance at this year’s tournament has earned you a scholarship from the National Individual Events Tournament of Champions. In order to receive this scholarship, you must be enrolled in a post-secondary endeavor.

Please return this form by October 1st of the year of your post-secondary enrollment. Checks will be payable to and sent directly to the institution in which you are enrolled. Requests must be submitted within two years of graduation.

IF YOU EARNED MORE THAN ONE SCHOLARSHIP, PLEASE COMPLETE THIS FORM FOR EACH OF THEM.

For all contests through 2019, the champions of all events, including the Perfect Performance, earned $750.

Beginning with the 2021 tournament, the following will be awarded:  1st place=$750; 2nd place=$400; 3rd place=$200; 4th place=$100; 5th place=$100; 6th place=$100. Perfect Performance Champion = $750.
