This is an absence request form for students of ISH, for absences more than 2 days.
Absence requests should be submitted at least 2 weeks in advance.

If the absence is 2 days or less, then do not use this form. Just submit an excusal request via Managebac with explanation. Once the form is approved, you are still required to write excusal (with reason) on Managebac so teachers are aware of the student's reason for absence.
