Registration form for the 2023 season.

This will be for all youth born in 2009-2016.

Baseball is based on age versus grade and depending on the numbers determines how the teams are divided.

T-ball registration and information will come out at a later date.

**Please complete a new form for each player in your family***

Travel Boys - $40 (7 to 8 year olds - includes t-shirt, weekly practices, and games)

Baseball - $150.00 + $200 Concession Fee separate check (9-14 year's, Digit, Bronco, Pony)

Softball - $ 150.00 + $200 Concession Fee separate check (9-18 year old's) 

All players will receive a new uniform this year and any new baseball or softball player will receive a hat or visor. 

***LATE Registration after February 20th is $200.00

**** There is a $200.00 Concession Fee. (WHY? We need volunteers to run the concessions and this is our main fundraiser for the season) This need to be paid at the time fees are paid. Your child's registration will not be accepted until we receive this deposit. Checks will held and deposited ONLY if you do not work otherwise all checks will be shredded at the end of the season.  Only one deposit per family is required.

I hereby certify that my child is in normal health and capable of safe participation in the 2023 Logan View T-Ball/Softball/Baseball Program. I assume all responsibilities in case of an accident at the facility. I hereby authorize the Logan View Ball Association to obtain medical treatment for my child in the event that parents and the emergency contact provided cannot be reached. I support the Logan View Ball Association philosophy, which is based on participation, fun, physical fitness and health, skill development, teamwork, fair play and volunteer leadership.
