Guidelines for submitting feedback:
Faculty, staff and students at the UM SOP may use this form to report curriculum related matters such as challenges with courses or concerns with individual or team dynamics.  In addition to this form, comments may be forwarded directly to the Associate Dean of Academic Affairs, department representatives on the Curriculum Committee and/or the P1-P4 Class Vice Presidents.  

Form submissions will be sent to an email account that is checked on a regular basis.  The Associate Dean of Academic Affairs office will respond within 5 business days.  All submissions should be professional and constructive.
