This form does not replace your emergency contact list/data collection information - it is simply for you to provide us with a list of people who you think might collect your child from school for you on the given date. It could include grandparents, aunts, uncles, your friends or parents of your children’s friends. It can be updated at any time in person or by sending in a new signed form. We will not take amendments over the telephone and will not permit children to leave with someone who is not showing on your list – so please ensure you fill out this form accurately and to submit the form again if there are any changes to keep it up to date.
