READ THESE INSTRUCTIONS ***You must be an approved Presenter to fill out this form. If you have not presented at Dave's Health & Nutrition before, please fill out the Presenter Application Form at https://goo.gl/forms/RZ632pSiRmeQ6YSM2, and wait for approval.

If you wish to submit a request for multiple classes, please download this form from our website at: https://daveshealth.com/wp-content/uploads/2019/04/CLASSROOM-RENTAL_your-name-here.xlsx, make payment, and submit it to classes.daves@gmail.com. We will contact you within 3-5 business days for final approval.

Deposits must be received by the 15th of the month 6 weeks prior -- pay by February 15th for April classes, by June 15th for August classes. Upon receiving this form, review may take up to 3 days. We will notify you by email or phone.

Class information will appear in our Print Newsletter, Class Calendar on our website and, if possible, our e-newsletter.

 If there is a cancellation, the deposit refund may be returned as deemed by management, depending on circumstances surrounding cancellation time.
