Our desire is to provide the best possible atmosphere throughout the trip. Students are expected to cooperate with staff at all times and to participate in all scheduled activities.

Possession and/or use of alcoholic beverages and/or any type of drugs (including vaping)  is strictly prohibited. Failure to remain within these guidelines at any time is cause for the student’s immediate to return home at the parent’s/guardian’s expense.

We have read the code of conduct and agree to abide by it.

I agree not to hold CAMPUS LIFE responsible for any injury that may result from my son’s/daughter’s participation in the CORE Retreat - November 22-23, 2019.

I also authorize CAMPUS LIFE STAFF to secure any emergency medical treatment necessary during the trip and I further assume all responsibility for the decisions so made.

I hereby grant permission to CAMPUS LIFE the right to use, reproduce, and/or distribute any photographs, film, video-tapes and sound recordings of my child.

Students must have the 2019-2020 Medical form completed before attending any Campus Life trip or event. Once completed, it covers all events/trips Campus Life may take during the 2019-2020 school year.
