8th Grade OWL Pre-registration Application

Thank you for your interest in the 8th grade OWL (Our Whole Lives) program at FUUSB!

Please complete this form to reserve a spot for youth in 8th or 9th grade this fall.

Four (4) steps are necessary to secure enrollment:
1) Complete this preliminary form
2) Complete the FUUSB Faith Development Registration form for the 2020-21 program year
3) RSVP to attend a mandatory parent orientation.
4) Commit a pledge/registration fee: FUUSB members and friends must have committed a financial pledge for corresponding OWL year. Non-FUUSB Members must pay a fee of $350.00, or have made alternate arrangements with Erika Reif, Director of Lifespan Faith Development.*

* Please contact Erika at 802-862-5630, ext. 2800 OR erika@uusociety.org if these fees present any hardship for your family; scholarship and payment plans are available, and ability to pay is NOT a factor in participant acceptance. Economic diversity is beneficial for all participants.
