I, the undersigned, fully empower and permit Rabbi Nissan Antine and/or Rabbi Cooper to act in my behalf to sell all Chometz possessed by me (knowingly or unknowingly) as defined by the Torah and Rabbinic Law (e.g. Chometz, doubt of Chometz, and all kinds of Chometz mixtures). Also Chometz that tends to harden and to adhere to a surface of inside of pans, pots or cooking and usable utensils and all kinds of live animals that have been eating Chometz or mixtures thereof.

Please state the locations where chometz can be found in your house on Pesach. Try to be as specific as possible. Even if you leave locations out, the sale is understood to refer to all of your chometz.

Rabbi Antine and/or Rabbi Cooper has the full right to sell and to lease by transactions, as he/she deems fit and proper and for such time which he believes necessary in accordance with all detailed terms and detailed forms as explained in the general authorization contract which have been given this year to Rabbi Antine and/or Rabbi Cooper to sell the Chometz. This general authorization is made a part of this agreement. Also do I hereby give the said Rabbi Antine and/or Rabbi Cooper full power and authority to appoint a substitute in his/her stead with full power to sell and to lease as provided herein. The above given power is in conformity with all Torah, Rabbinical regulations and laws, and also in accordance with the laws of the State of Maryland of the United States. And to this I hereby affix my signature on this date in the year 2024. I understand that by submitting this form, I am agreeing to the sale of my chometz per the stipulations above.
