2024-25 SP4K Enrollment Form- STEP 1      
THIS IS A REQUIRED STEP FOR ENROLLMENT IN THE  SP4K PROGRAM. Additional site specific paperwork must also be completed prior to attendance.

Completing the form does not guarantee that a spot will be available at the location you have selected as your first preference.   SPASD and SP4K reserve the right to place students.   Placement letters notifying families of their child's site and class time will be mailed out by mid-June.

ALL SP4K students are enrolled in a 5 day per week program, Monday-Friday, district attendance policies and procedures will be followed. The SP4K program is a one-year program and students are expected to enroll in kindergarten the following year.  

Wrap around childcare services ARE NOT part of the SP4K program. Arrangements for wrap care, if needed, must be made with the individual site.  Acceptance into the 4K program does not guarantee wrap care availability or vice versa.

If transportation needs change the student's site placement may change.  
Transportation is NOT provided for wrap-around care.

Transportation:        If your child will need bus service to and/or from their SP4K program they will be enrolled in a program that is located within their assigned bus route area. Bus route areas are based on the address used for pick up and drop off which must be the same.      The district and the bus company will determine your child's time of day and site placement based on their busing needs.
