This is the booking form to attend the Gloucestershire Sight Loss Council VI forum on Health and Social Care. The meeting will take place virtually via Zoom, from 11:00 to 13:00, 01 December 2020.

If you would like to ask the panel a question, please submit your question when filling in our short booking form below. If your question is not asked during the event, you will get an answer in writing from the relevant speaker after the meeting. Within 24 hours of submitting this form, you will receive an email confirming you have registered successfully.

Seven days before the event, we will email you with the Zoom link to join the meeting and the telephone number if you wish to dial in. You will also get the final programme for the event and any presentations that are to be used.
