NOMINATION FORM
All online nominations are submitted electronically to music@cqu.edu.au.  Upon receipt and acceptance of your nomination for the CQ Schools’ Jazz Festival, your school will receive an email detailing the instructions for online payment.  This payment is made directly to CQUniversity.  For any additional information regarding nomination, participation or payment, please contact the CQSJF Festival Administrator on 49407800 or music@cqu.edu.au.

FEES
Nomination Fee: $330.00 (inc. GST) per ensemble
Multiple Ensembles: $330.00 (inc. GST) for your first ensemble, $220.00 (inc.GST) each additional ensemble.

NOMINATIONS 
All nominations close Friday 21 June, 2024

PLACES ARE LIMITED
Priority is given to the first ensemble of each school BEFORE accepting the nomination of multiple ensembles from any one school.
