This form should be used to communicate Intention to withdraw a currently enrolled student.
This form can only be used for ONE student. (If you intend to withdraw multiple students, please submit this form multiple times.)
Please be reminded that in accordance with the Continuous Enrollment Agreement, this form must be submitted before March 31 to avoid late withdrawal fees. Forms submitted after March 31 will be subject to the fees outlined in the Continuous Enrollment Agreement.
