The purpose of this form is to get information on any dietary or health requirements during the camp. Please ensure you list all dietary requirements, any allergies, and any medical conditions. Dietary requirements and allergy information will be shared with the catering team and medical conditions and allergy information will be shared with the teams running the different activities.
