The Sports Vouchers program is a Government of South Australia initiative administered by the Office for Recreation, Sport and Racing. The program provides an opportunity for primary school aged children from Reception to Year 9 to receive up to a $100 discount on sports or dance membership/registration fees. The purpose is to increase the number of children playing organised sport or participating in dance activities by reducing cost as a barrier. By submitting this form, you give permission to the Woodville Lacrosse Club to share your information with the Office for Recreation, Sport and Racing. WLC will submit your details on your behalf, and deduct $100 from your child's playing fees for 2023. More information on playing fees at www.warriorslax.com/info/subs.

Please note, a Sports Voucher may only be used once per eligible child per calendar year. If a Sports Voucher has already been claimed for your child for another activity in 2023, you will not be eligible to claim a Sports Voucher for lacrosse as well.
