Caddo Mills Community Sign-up for COVID-19 Pfizer Vaccine at CMISD

CMISD has collaborated with US Medincineers in Plano, Texas to provide a COVID-19 vaccination clinic. This form is intended to collect information for individuals that would like to receive the Pfizer COVID-19 vaccine. The Pfizer vaccine consists of two doses This vaccine will be available to Caddo Mills ISD, Caddo Mills Community, Boles ISD and Bland ISD faculty/staff/family, if choosing to participate. The clinic will be held May 5th, 2021 from Noon - 5:30 p.m. at Ed Locker Stadium located in Caddo Mills, Texas. You must bring a valid ID to your appointment time. There may be additional paperwork to be completed upon arrival at the vaccine clinic.
