We take health and safety seriously at SMASH!. In order to volunteer at SMASH! 2022, you will be required to read through our policies prior to our convention days and acknowledge that you will abide by the requirements set out in these documents.

All information gathered in this form will be kept strictly confidential.

Upon successful submission, you will receive a copy of your response from Google Forms, this will be sent to the email provided, however may land in your junk / spam folder, be sure to double check for it there.
