Custom T-shirt Order Form
Add a custom design to any cotton t-shirt, the possibilities are endless!

PRICES:
Adult designs start at $30 each
Kids designs start at $20 each

NOTES:
--- Requests must be made MINIMUM TWO WEEKS IN ADVANCE of the date for pick up. (EX: 2 weeks of time for me to work, starting the day you order your shirts & ending the day you need to pick up).
--- Any thing less than 2 weeks is considered a RUSH ORDER and will be an additional $10 per shirt.
---  Orders will not begin until payment is complete.
