Women in Wellness Awards 2024 - Nomination Form
Greetings!

Sista Afya Community Care will have our 2nd Annual Women in Wellness Awards that will honor women across Chicago making a difference in the field of wellness on Saturday, October 26, 2024 at Room 1520. 

Nomination Guidelines:

  • Must live in the Chicagoland area

  • Be ages 18 or older.

  • Identify as a woman.

  • Upholds values of integrity, provides and promotes quality of care in the wellness field, and has served women in Chicago.

  • We will consider all nominations, however we are especially looking for women who have demonstrated consistent impact in the wellness field that has transformed and improved the lives of people in Chicago.

  • Person must be nominated by someone else. There are no self-nominations.

We are looking for our Chicago community to nominate women in their communities for our awards in the following categories:
  • Mental Wellness
  • Physical Wellness
  • Holistic Wellness
  • Spiritual Wellness
  • Community Wellness
  • Wellness Woman of the Year

Mental Wellness: Woman who supports the emotional and mental wellbeing of others.

Physical Wellness: Woman who promotes and supports others with taking care of their physical bodies.

Holistic Wellness: Woman who  addresses the physical, mental, emotional, social, and spiritual components of health.

Spiritual Wellness: Woman who supports others to recognize something greater than yourself and having a set of values, principles, morals and beliefs that provide a sense of purpose and meaning to life, then using those principles to guide your actions.

Community Wellness:  Woman who advocates for the combination of social, economic, environmental, cultural, and political conditions that are essential for people flourish and fulfill their potential.

Wellness Woman of the Year: A woman who has gone above and beyond to improve the well-being of women in the Chicago community.
