依照 NSW 社區語言學校部規定, 每位學生都需要填一份報名表。請務必確認報名表上的資料是正確的 (如: 英文姓名，就讀的NSW主流學校，學校名稱，學生出生年月日，等等 )。 STS將呈報學生資料到 NSW Education Department。每張表格只代表一位學生，填完回傳即可，謝謝。

As per requested by NSW Community Language School Department, each student is required to fill in a copy of the enrolment form. Please ENSURE the information is accurate and MATCH the mainstream school your child attends locally. We will then submit to NSW Education Department. Please complete the form, one record per student, thank you.
