FOPT Mentorship Intake Form
Welcome to the Finding Our Power Together Mentorship Program! We can't wait to work with you in support of your wellness goals. Mentorship can be whatever you want it to be but some examples of what can work on is cultural identity and approaches to wellness, mindfulness practices, self-care skills, goal setting, habit formation, stress management, academic/professional support, or anything else you would like to explore!

The mentorship program is for all self-identifying Indigenous youth (ages 14-29) from across Canada who are seeking support in their life journey. This program is free and fully accessible online and/or through telecommunication.

This form will provide us with some background information to help us get you matched with a member of our mental health team and guide your initial conversations. Feel free to provide as much or as little information as you feel comfortable with; you can skip any questions you don't want to answer, just make sure you go to the end and hit submit. We will get in touch with you soon after using the information you provided!
