I hereby give the Church of the Little Flower, and all persons acting with its permission, the absolute right and unrestricted permission to obtain, use, copyright, and/or publish photographic portraits or pictures of the above named person(s), whether such pictures are still, moving, single or multiple, or in which the above named person(s) is/are in whole or in part, in conjunction with the person’s own name or another fictitious name. It is my understanding that such picture(s) are for the purpose of internet publication on the church website, art, advertising, and any other lawful purpose whatsoever. I understand further that I will not have an opportunity to approve or review the finished product that may be used in connection therewith or the use to which it may be applied. I agree to withhold Little Flower, all volunteers and all sponsors, their representatives, employees, and successors, individually or otherwise, from any claims, damages, actions, or liabilities of any kind relating to the public display of my child’s name, and if applicable, accompanying photograph, and event information, in any broadcast, print or any event for promotional, publicity or other legitimate purposes. I have fully read this waiver and publicity release form and understand its terms. 

I agree and understand that by signing the Electronic Signature Acknowledgment and Consent Form, that all electronic signatures are the legal equivalent of my manual/handwritten signature and I consent to be legally bound to this agreement.
