COVID-19 REQUIREMENTS FOR CITY OF AUBURN PARKS, RECREATION FACILITIES AND ATHLETIC FACILITIES

 Participant has not traveled out of the country in the last 14 days.
 Participant has not been experiencing a cough, difficulty breathing or a loss in taste/smell.
 Participant does not have a fever of 100.4 degrees Fahrenheit or above.
 Participant has not been in contact with anyone who has a confirmed case of COVID-19 in the last 14 days.
 Participant agrees to stay home if they are sick.
 Participant agrees to sanitize hands upon entry into the facility.
 Participant agrees to exit the facility immediately upon completion of their game.
 Participant will be required to be 6 feet apart when on the sidelines.
 Participant agrees not to use the facility’s water fountain.
 Participant is required to wear masks at all times (such as in the bench area), except when they are directly
participating in the activity on the field of play. All participants must provide their own masks.
 Participant will not be allowed to share equipment. Players must provide their own helmets, bats, and gloves.

I acknowledge that I have read and understood the City of Auburn, Alabama’s Athletic participation requirements.

I agree to follow these requirements as a condition to my child’s participation in the athletic event taking place at/on the City of Auburn park, facility, or athletic facility. I acknowledge and understand that my failure to abide by these reservation requirements may result in cancellation of my reservation. I acknowledge and understand that adherence to the athletic participation requirements in no way guarantees that my child will not be exposed to COVID-19 infection.

The City of Auburn does not guarantee that no one will be exposed to COVID-19.
