小朋友的生死教育調查

你好，我們是香港理工大學設計學院四年級學生。

非常感謝你在百忙之中抽出寶貴的時間參與是次問卷調查，問卷調查的主要目的是想了解香港家長對於小朋友的生死教育的看法。

此問卷僅供學術研究和參考討論，絕不會向外公開，問卷以不記名的方式進行，如有收集到任何個人資料，將絕對保密並在學術研究後摧毀，確保受訪者的資料不會外洩。

Hello, we are year 4 students of the School of Design, Hong Kong Polytechnic University.

Thank you very much for taking the time out of your busy schedule to participate in this questionnaire survey. The main purpose of the survey is to find out the views of Hong Kong parents on the education of children's life and death.

This questionnaire is only for academic research and reference discussion, and will not be released to the public. The questionnaire will be conducted anonymously. If any personal information is collected, it will be kept strictly confidential and destroyed after academic research to ensure that the data of interviewees will not be disclosed.
