MATAI Membership Application
Please fill out the following form for review by the MATAI Board.  Membership review / approval process could take up to several weeks depending on the schedules of the board members and the voting process.  Upon approval you will be provided a link to submit your $45.00 annual membership fee.  This fee can be paid via the link online or if needed an invoice can be sent to you or your employer.
