Locker Request Form - 2023-24

By completing this request form, you are agreeing to the following guidelines:
- This locker is the property of the Upper Grand District School Board
- Take care of your locker. You will be responsible for emptying and cleaning it at the end of the year.
- Students will be required to pay for any damage done to their assigned locker.
- Lockers may not be shared. Each student should have their own locker. You are responsible for items in your locker. You must remain in the locker assigned to you.
- Any damage to lockers should be reported immediately to the office.
- Locker decorations may be taped to the inside walls or inside the door. Decorations may not be displayed on the outside of the locker. Do not use marker or ink of any kind (including dry erase markers) in lockers.
- Items which may be considered offensive or inappropriate must not be posted.
- Lockers may be opened for emergency purposes or other reasons as required by the school Administration or other authorized Upper Grand District School Board employees.
- Perishable food items, items which could potentially be deemed unsafe or cause harm to self or others, alcohol, controlled substances or any other item prohibited by Upper Grand District School Board may not be stored in a locker.

To receive your locker assignment, complete the following information. We will contact you with your locker information. If you no longer require use of a locker, please notify the office when the locker has been vacated.

Are you requesting a locker in the Grad hall? (for Grade 12 students only) Note: there is a limited number of grad hall lockers available. They will be issued to Grade 12 students on a first come, first served basis.

Do you need to purchase a lock? Locks are $6 and may be purchased through your school cash account or by using the link below. If you purchase your lock through the school cash program, we will send you an email with your locker number and the combination to the lock that is currently on the locker. All lockers currently have a lock on them. If you wish to bring your own lock, you will need to get the combination from the office and return the lock and the combination to the office. Locks will be available for purchase through your SchoolCash at the end of August. You may also purchase one with cash from the front office during summer office hours.
