Fern Hill Primary School Tree Tops HOME SCHOOL AGREEMENT 2021/2022
The Tree Tops Home School Agreement sets out the commitments made by both the school and the parents or carers of children attending Tree Tops to ensure its safe and effective operation and includes the terms and conditions of booking a place at the provision.

This form provides for the collection of important information about your child and acts as confirmation that you have read the Tree Tops Home School Agreement commitments and terms and conditions of booking.  

If you have more than one child attending Tree Tops, you must complete and submit a separate form for each child.

When you submit this form, a copy of your completed form will automatically be sent to your email address.

Section 2 asks for information about your child, including parental and emergency contact telephone numbers and medical and dietary details. It also asks for information about who is authorised to collect your child from a Tree Tops session.

Section 3 is a declaration that you have read and understood the Tree Tops Home School Agreement commitments and agree to the terms and conditions of booking.

We ask for a Home School Agreement to be completed every new school year. This is to ensure we have up to date emergency contact and medical details for every child. During the school year, it is the responsibility of a parent or legal guardian to inform us of any changes to this information. Changes must be notified in writing to Tree Tops directly by email at: treetops@fernhill.rbksch.org
