St Albans Hockey Club Camp Booking Form
Please use this form to book our Hockey Camp! It will be led by a team of qualified coaches for ages 8 - 18 years.

£95 - 2 Days
£135 - 3 Days

Bookings & Cancellations:
Although restrictions are lifting, there will be a limited number of places in each camp so bookings will be made on a first come first serve basis.
As a precaution, temperatures will be taken upon arrival. If your child has a temperature of 38 degrees or above, you will be asked to take them home.

To book a place on the camp you will need to:
- Read the camp guidelines in full before making a booking.
- Fill out 1 booking form PER CHILD.
- Once the form has been filled, please send a TEXT MESSAGE to Dillet Gilkes on 07500 554422 with the following information:
* Name of child
*Day/s required
- Please wait for a CONFIRMATION TEXT with payment details attached. Please allow a few days for this text to come through.
- If you have to cancel your place at the camp, please provide a minimum of 24 hours notice if possible.

Payments:
- Please make sure you have confirmed you place with Dillet Gilkes before making any payments.
- There will be no pay on the day option.
