Please read the information below before completing this form.
To reserve your spot, you must authorize an automatic, non-refundable $60 deduction per month, per child, from your FACTS account.
The $60 covers 10 rides per month.
After 10 rides in one month, additional rides will be billed to your FACTS account, $6.00 per ride.
Parents are responsible for notifying Paola Londono in the Business Office if your student(s) will not ride the bus during a specific month.
Once you complete this form, you will receive a document to authorize St. Mary's Academy to charge your FACTS account.
*Bus routes will be determined based on rider interest and may be eliminated if ridership declines.
