CHMS SY23-24 Recommendation Letter Request Form
Recommendation Letter Request Policy: If you are applying out of Brooklyn Prospect for high school or would like to participate in a community/ academic program, you may require recommendation letters from your teachers, school counselor, and/ or principal. In order to ensure a fair amount of time for the teacher to produce a quality recommendation letter in time to meet deadlines, please follow the following policy/ steps:

1. If you would like to request a teacher/ school counselor/ principal recommendation letter(s), please FILL OUT THE FORM FOR  EACH TEACHER/ FACULTY member you would like a recommendation letter from.

2. We ask that you put in the request one month before the deadline that the recommendation letter is due. WE WILL ONLY ACCEPT REQUESTS THAT ARE AT LEAST 2 WEEKS PRIOR TO THE DEADLINE.

3. You will receive an e-mail confirmation that the request has been received. You will receive a recommendation letter via e-mail from the teacher directly or the school counselor once it is complete.
