Meals are being provided for all children  0-18 years of age, free of charge.
Sign up is needed to ensure we are prepared for the amount of meals needed.
Students may only receive 1 breakfast and 1 lunch per day.  If students receive meals in a school building from food service, they can not also receive pick up meals for the same day.

Number of children needing 5 days of meals? (example: Full Virtual, Cave, Preschool, 0-18 not in school).  "0" if none. *
I will be picking up at the following location.  Your information will be sent to that specific school .  Please call the food service office if you need to change location after submitting the form.  Only one location can be chosen.   *
My student has dietary restrictions.  A medical form is required for meal substitutions.   *
I understand that children may only receive one breakfast and lunch per day from CASD Food service. *
A copy of your responses will be emailed to the address you provided.
