In order to best serve our students and your child, the School Counseling Department would appreciate your feedback.  Your responses will only be viewed by your child's school counselor.

This form has listed programs and focus topics that our School Counseling Department has in place. These programs and topics are presented to your child through classroom lessons, small groups, and/or individual counseling. Your feedback helps the School Counseling Department develop goals for the year and helps your child with their academic, personal, social, and emotional success.

If necessary, you may complete a separate needs assessment for each of your students who currently attends HMS.
