This form is necessary for all seniors in order to gather their name for their diploma and to give authorization for sending transcripts.

Diploma:

In anticipation of Blue Valley Southwest’s 2023 Graduation, please complete this form indicating the way you would like your student's name to appear on their Blue Valley Southwest Diploma. We would like to suggest parents and students make this decision together. Most families choose to use first, middle and last name as it appears on your birth certificate. However, some elect to use a variation with only a middle initial. Our only restriction is that no nicknames should be used. If corrections are necessary, there will be a reprinting charge.

Authorization is required by the Parent/Guardian before any transcripts can be sent for their student.
